Anterolateral to the talus

The posteromedial approach is employed in order to expose the posterior talar body or the posterior process of the talus.
Posterolateral to the hindfoot
The posterolateral approach can be used for acute posterior talar fractures and non-union surgery.
Medial malleolar osteotomy
An osteotomy of the medial malleolus exposes the medial aspect of the body of the talus and allows the surgeon to protect the posteromedial deltoid branches from the posterior tibial artery.
