Place the patient in a supine position with the hip flexed 50° and the knee flexed 90°.
Commercially available carbon triangles can be used to support the leg during retrograde nailing of the femur. This allows easy access to the intercondylar notch.
Place a small bolster underneath the right buttock to minimize external rotation of the lower extremity.
Note: Carefully place padding underneath the knee area and proximal fibular region, in order to avoid compression on the popliteal vessels, sciatic nerve and common peroneal nerve. This padding can be easily provided by placing folded towels on the carbon triangle.
