Main indication: Fractures with a readily discernable gap, or step

Skill: Some specialized surgical experience
Equipment: Simple surgical and imaging resources

Indications

  • Fractures with a readily discernable gap, or step (a sign of potential instability)
  • Secondary displacement after initial closed treatment

Contraindications

  • Undisplaced fractures

Advantages

  • Stable fixation
  • Uncomplicated healing
  • Anatomical reduction

Disadvantages

  • Pin-track infection if K-wires are left protruding
  • Second anesthesia if K-wires buried
  • Necrosis of the capitellum following extensive (posterior) exposure, which can jeopardize the already vulnerable blood supply to the capitellum
