Cast fixation without reduction

Main indication: Stable, minimally deformed/displaced ulnar fracture, undisplaced radial neck fracture

Skill: Basic surgical experience, no specialized skills
Equipment: Basic equipment only

There are two requirements for treating this fracture closed:

  • Undisplaced radial neck fracture or nearly anatomical reduction possible
  • Stable ulnar fracture, length maintained in a molded cast

A well-molded cast and radiographic follow-up are recommended to ensure maintenance of satisfactory fracture position. If acceptable position cannot be obtained and maintained, operative stabilization is indicated.

Contraindications

  • Any instability

Advantages

  • No anesthesia
  • Later change of treatment possible

Disadvantages

  • Poor functional results especially close to skeletal maturity
  • High rate of displacement
