23r-E/1.1 Salter-Harris I

Epiphyseal fractures – isolated radius

23r-E/1.1 Simple epiphysiolysis, Salter-Harris I, radius

The mechanism may involve traction/rotation, or a fall onto an outstretched hand.

This injury is associated with local tenderness over the distal radius. Local swelling is not usually prominent.

Note: Be aware of nonaccidental injury in infants.

Careful scrutiny of the x-rays is important to identify the presence, or absence, of a metaphyseal component, as this may be the only radiologically visible sign of a corner/bucket handle fracture which can be associated with nonaccidental injury.
