Epiphyseal fractures – isolated ulna

23u-E/1.1 Simple epiphysiolysis, Salter-Harris I, ulna

This uncommon injury may result from traction/rotation, or a fall onto an outstretched hand.

This injury is associated with local tenderness over the distal ulna. Some local swelling is usual.

Note: Be aware of nonaccidental injury in infants.

Careful scrutiny of the x-rays is important to identify the presence or absence of a metaphyseal component, as this may be the only radiologically visible sign of a corner/bucket handle fracture which can be associated with nonaccidental injury.
