Epiphyseal fractures – isolated ulna

23u-E/2.1 Simple epiphysiolysis with metaphyseal wedge, Salter-Harris II, ulna

This is an uncommon fracture that is caused by a fall onto an outstretched hand.

This fracture is associated with local tenderness over the distal ulna. Local swelling is usual.

Check for signs of ulnar nerve deficit.

Note: Be aware of nonaccidental injury in infants with a very subtle metaphyseal component.

X-ray shows the fracture, direction of displacement and the size of the metaphyseal fragment.
