Posterior injury
Both posterior arch disruptions lie outside the sacrum.

This is classified as an AO/OTA 61C3.1 injury.

Anterior injury
The anterior injury is one or more of the following:

  • Symphyseal disruption
  • Pubis (uni/bilateral, ipsi/contralateral)
  • Ramus (uni/bilateral, ipsi/contralateral)

Bilateral unstable pelvic ring injury, one side sacral and contralateral extrasacral  

External rotation injury

Posterior contralateral injury

Anterior injury

On one side there is a complete vertical fracture through the sacrum. On the other side there is a fracture through the ilium.

This is classified as an AO/OTA 61C3.2 injury.

Anterior injury
The anterior injury is one or more of the following:

  • Symphyseal disruption
  • Pubis (uni/bilateral, ipsi/contralateral)
  • Ramus (uni/bilateral, ipsi/contralateral)

Bilateral unstable sacral fractures

Posterior injury
Both posterior injuries are complete vertical fractures through the sacrum. These include complex lumbosacral dissociations and suicidal jumpers' fractures. (See sacral fractures)

This is classified as an AO/OTA 61C3.3 injury.

Anterior injury
The anterior injury is one or more of the following:

  • Symphyseal disruption
  • Pubis (uni/bilateral, ipsi/contralateral)
  • Ramus (uni/bilateral, ipsi/contralateral)
