General considerations


Transverse fractures are typically due to high energy forces. The risk of displacement, angulation, and slow healing is increased. Associated fibular fractures increase instability.
If available, closed intramedullary nailing, or occasionally plate fixation are often preferable to nonoperative treatment. If a satisfactorily aligned closed reduction is obtained, angulation can usually be controlled in a cast, so that this option is worth considering.
