Fixation of the lesser tuberosity including anchorage of the subscapularis tendon is primarily done with sutures placed through the tendon insertions. Provisional reduction and stabilization are accomplished with such a suture. Definitive fixation should include a secure suture to appropriate plate holes.
Supplementary fixation of the lesser tuberosity with lag screws or absorbable polymer pins might be considered. If the stability of lesser tuberosity fixation is questionable, it can be checked clinically by rotating the arm. If there is any micro movement visible or palpable one should consider additional fixation.
